For New PatientS

Once you schedule your first appointment, you will be emailed forms to complete including a medical history form, consents to treatment, and HIPAA policy. Please fill out your new patient paperwork to the best of your ability at least 24 hours prior to your 1st appointment. This will save time during our discussion at your first visit, and will leave more time to focus on your initial treatment. Please also collect and bring along any relevant diagnostic imaging (i.e. X-rays, MRIs, etc.) or blood labs that you would like for me to review.

What should I wear to my First Osteopathic Appointment?

The Osteopathic treatment is performed over the clothing. Wear clothes that are comfortable and loose-fitting. Avoid wearing jeans and bras with underwire if possible. You may be asked to remove hats, hair clips or ties, glasses, belts, bulky items in your pockets, or shoes.

Directions for your appointment

Please arrive 5-10 minutes early for your appointment. You may park in the parking area to the left side of the building. Please enter through the front door or, alternatively, there is a wheelchair ramp to enter through the back door if required. Please wait quietly in the seating area on the 1st floor. There is a restroom in the waiting area you may use. Dr. Levi may be with another patient, but will meet you in the waiting area at the start of your appointment time.

A Note about Fragrances/ Perfumes

Please refrain from wearing perfumes, using products with strong fragrances, or smoking prior to your appointment as this can affect our patients with chemical sensitivities and/ or respiratory issues.

A note about Childcare

If you have young children who may have difficulty being patient while you are being cared for, I encourage you to arrange childcare in order to receive the best treatment possible.

Resident or Student Observation

Dr. Levi works with trainees in the Osteopathic profession on occasion. A student or resident may be present during the time of your visit for the purpose of furthering their understanding of Osteopathy and to develop their palpatory skills in diagnosis and treatment. All learners are required to sign and adhere to a strict privacy policy. The addition of a learner can greatly enhance your treatment experience. That said, your health and needs are of the utmost concern. If you do not wish to have a learner observe or participate during your appointment, please let Dr. Levi know.

Important Note about Late Arrivals

If you arrive late to your appointment, your appointment will end at the scheduled time resulting in a shortened appointment time.  If you arrive late by 10 minutes or more of your appointment time, you will be asked to reschedule and return at another time.

In case of Emergencies

If you experience a medical emergency, please call 911 or seek care at an emergency department. Dr. Levi does not provide emergency healthcare services. She is considered to have an outpatient, specialty practice in Osteopathic Neuromusculoskeletal Medicine. You must maintain your own primary care provider for assistance with your urgent and routine health needs.
