Pricing and Payment

  • Does Dr. Levi accept my insurance?

    Dr. Levi is out-of-network for all insurances except Medicare (see below). This means your insurance will not be billed, and you are responsible to pay the entire charge for your visit at the time of service. As a small practice, this model allows us to provide the best care possible with longer appointment times while maintaining lower overhead.

  • Does Dr. Levi accept Medicare?

    Dr. Levi is in network with Medicare but not with Medicare Advantage plans. It is important that you check your insurance prior to your appointment to determine which plan you have. Additionally, Dr. Levi is a non-participating provider with Medicare, and she does not take assignment from Medicare at her private practice. This means Dr. Levi does not collect payment from Medicare, and you are responsible to pay the entire charge for your visit at the time of service. Dr. Levi will submit a claim on your behalf to Medicare, and you may receive partial reimbursement from Medicare as well as your supplemental insurance. However, it is your responsibility to follow up with Medicare and your insurance regarding your reimbursement if needed. A Superbill can be provided following your appointment for you to reference if you need to follow up on your claim that has already been submitted.

  • How do I check if my insurance will reimburse me?

    In some cases, your insurance may reimburse you or may apply your payment towards your deductible in part or full. Dr. Levi cannot make this guarantee, and you are responsible to check with your insurance prior to your appointment. The codes that are typically billed for your appointment are Evaluation & Management (EM) Codes: 99205, 99214 and Current Procedural Terminology (CPT) Codes: 98927, 98928, 98929. Following your appointment, a Superbill will be provided for you to submit to your insurance upon request.

  • What Is the Pricing of an appointment?

    Dr. Levi values full transparency of pricing so you can plan for your healthcare needs. Charges are reduced from insurance pricing and are as follows:

    Adult Initial Evaluation: $300

    Adult Established Follow-up Appointment: $180

    Pediatric Initial Evaluation: $250

    Pediatric Established Follow-up Appointment: $165

    Home visits may be available under certain conditions. You may contact us to discuss further.

  • What method of payment is accepted?

    Dr. Levi accepts cash, personal check, and Visa or Mastercard credit cards. You may also use your Health Savings Account (HSA) or your Flexible Spending Account (FSA) if you have one. When you complete your intake paperwork, you will be asked to enter your credit card information to hold your appointment, but you will not be charged until after your appointment. You may use a different payment method you prefer at that time. Please make checks out to “Kendra Levi DO, LLC”. A $35 service charge will be assessed to your account for any returned check.
